TLM 1 - Setup

General notes

• • Room and equipment setup are crucial to a successful case.

    • As the surgeon, you must have everything ready and know how to use it

    • Work with Scrub tech to set up according to these pictures

  • Microscope

    • As part of your pre-surgery checklist, balance and arrange the microscope 

    • Place the assistant's viewfinder to the right

    • Connect the Black Diamond HDMI cable to allow video monitoring and photos 

  • Request Neptune Suction for the case

All cases

• The room layout

  • • Bed turned 90 degrees counter clockwise

    • Video monitors directly in front of the surgeon at eye level and off the left at the assistant's and scrub tech's eye level 

    • Light box behind your left shoulder

    • Microscope behind your back and left

    • Laser behind your right shoulder

      • Laser Setting for glottis: 5W, 0.10s repeat accublade with micromanipulator

      • Laser Setting for Tongue Base: 20W continuous

    • Assistant stands / sits to the right of the surgeon, passing equipment as needed

    • Foot pedals (right foot only)

      • Laser pedal on left

      • Bipolar pedal on right

• Head of bed

  • • Zietels suspension laryngoscopy system at level of patient's ear on right side of head of bed

      • Zeitels system is not very intuitive.  You may have trouble setting it up for the first time

    • Be careful with placement of light carrier with Zeitels laryngoscope.  It can get very hot and may be close to the patient's face.  Prolonged contact can cause burns.

    • Suction holster on head of bed between surgeon's legs 

    • Split sheet tucked under shoulders

    • Head drape wrapped over the upper face
