Anterior canaloplasty
Work lateral to medial
Maintain a medial based skin flap
Remember "Skin is gold"
Protect the sky flap with foil
Drill lateral to medial with 3-4 diamond; use copious irrigation to limit heat damage
Curette the medial ledge
Posterior canaloplasty
Be more conservative
Avoid the mastoid air cells
Be aware of an anterior facial nerve; use the monitor
Atticotomy
May extend to expose the epitympanum; separate IS joint and tensor tympani
May lateralize the malleus and incus and reconnect with Otomimix
